SH5053 : MDT Effectiveness Report

SH5053 Summative Assessment 1 A: Guidance for 2000 words group report (80% of overall grade).

Scenario

You are a team that has been set up within South Rotherton as part of the recently introduced Integrated Care System (ICS). Your goal is to develop the effectiveness of multi-disciplinary teams within the neighbourhoods that form the place of South Rotherton.

Your team has been asked by the Health and Care Partnership of the ICS to write an interim report on your work in developing effective teamwork. This should identify local challenges as well as those identified in the research literature and should make recommendations designed to deliver an integrated workforce culture focused on patient centred care.

Students should follow the main structure outlined in the template on WebLearn.  It is a compulsory that this template is used to complete the work.   

Executive Summary (approximately 150-200 words)

Brief summary/overview, outline key problem & objective[s], main findings & key recommendations

Introduction (approximately 250-300 words)

Reflects structure of report, sections/subsections, background information, terms of reference [who commissioned, scope], define key terms

Main body (approximately 1000-1100 words

Methodology

How was the information collected?  Look at the weekly slides where it is explained about having meetings with the team at South Rotherton

Literature review on working in MDT’s.  Refer to the resources provided in the folders for the weekly sessions as well as wider reading.

• What is an MDT?
• What are the challenges/barriers working in an MDT?
• What are the implications of ineffective MDTs?

Results/findings (Look at weekly slides where it mentions about challenging statements made – look at the ‘assessment workshop’ slides)

• Objective description.
• What did you find out about the MDT in South Rotherton?
• What are the challenges?
• What did you find out may be the reason for the challenges?

Discussion:

• Critically evaluate what you found.
• Use the literature from the module to discuss what you believe the MDT at South Rotherton needs to do to be more effective

Summary/conclusions (approximately 200-250 words)

• overview of main findings
• demonstrate achievement of aims
• refer to themes/issues from introduction
• summarise findings presented
• should be able to understand the major issues, points, and findings from this section alone

Recommendations (approx. 200 – 250 words)

Use literature and make recommendations designed to deliver an integrated workforce culture focused on patient centred care

SH5053 Summative Assessment 1 B: Guidance for 500 words individual reflection (20% of overall grade)

Students are required to reflect on the process of working as part of a group to produce the report.  Here you are urged to use a reflective model to complete this task. Click here for website on reflective models.

Students should attend to correct academic presentation of their work. Work should be presented in report format, should have a clear structure with an introduction and conclusion. Correct academic presentation includes double spacing your work and providing page numbers. Attention to carefully editing work for grammar and spelling. Students must follow the Harvard referencing system.
